reordercontacts.com is operated by Eyefinity, Inc. to facilitate the sale of prescription contact lenses by independent registered eye care practitioners (an "ECP") to their patients ("you"). In order for you to purchase contact lenses through this Site: (a) your eye care practitioner must be registered with Eyefinity, (b) your ECP must have registered you to use the Site (c) your ECP must have provided your prescription information to ReorderContacts.com; and (d) you must be at least 18 years of age or between the ages of 13 and 18, provided that if you are between the ages of 13 and 18, your use of this Site must be supervised by your parent or legal guardian. The Terms set forth herein govern your use of the Site and your purchase of contact lenses from your ECP. When you access and use this Site, you agree to these Terms, including the Privacy Policy. You ratify your agreement to be bound by these Terms when you purchase contact lenses from your ECP through or by otherwise using or accessing the Site and confirm that any such purchase is made in connection with treatment provided by the ECP that registered you and/or that you have consented to your ECP disclosing your prescription and related information to Eyefinity and its suppliers and service providers as necessary to facilitate the fulfillment of your order. |
